Community Liaison
The Community Liaison is responsible for building and maintaining referral relationships, increasing brand awareness, and driving new client admissions for the home care agency. This role combines sales, marketing, and community outreach to promote the agency’s services to community organizations, and potential clients. The ideal candidate is outgoing, relationship-driven, and motivated by growth and results.
Key Responsibilities
Business Development & Sales
- Build and maintain strong referral relationships with case managers, social workers, hospitals, clinics, nursing facilities, assisted living communities, senior centers, and community organizations.
- Actively generate new referrals and leads for home care services.
- Follow up on referrals and collaborate with intake/scheduling teams to support timely admissions.
- Track referral sources, outreach activities, and outcomes to support growth goals.
Marketing & Community Outreach
- Represent the agency at community events, health fairs, senior expos, and networking meetings.
- Conduct in-person visits to referral partners to present agency services and value proposition.
- Distribute marketing materials (brochures, flyers, business cards) and ensure consistent brand messaging.
- Assist in planning and executing community engagement initiatives and promotional campaigns.
Relationship Management
- Serve as a primary point of contact for referral partners and community stakeholders.
- Maintain ongoing communication with partners to ensure satisfaction and strengthen long-term relationships.
- Address concerns or feedback from referral sources in coordination with leadership.
Reporting & Documentation
- Maintain accurate records of outreach efforts, referral activity, and contacts in tracking system.
- Provide regular reports on performance metrics (referrals generated, visits conducted, partnerships developed).
- Support leadership with market insights and opportunities for growth in the service area.
Qualifications
- High school diploma required.
- Previous experience in sales, marketing, healthcare outreach, or community relations preferred.
- Strong communication and interpersonal skills; comfortable with face-to-face outreach and relationship building.
- Self-motivated, goal-oriented, and able to work independently.
- Basic computer skills (email, Microsoft Office/Google Workspace).
- Valid driver’s license and reliable transportation (field-based role).

Compensation & Benefits
- $15 per hour performance-based incentives/commissions of $250
